  • Diagnosed with an inflammatory disease: ankylosing spondylitis (AS), Crohn's disease (CD), hidradenitis suppurative (HS), juvenile idiopathic arthritis (JIA), psoriasis (Ps), psoriatic arthritis (PsA), rheumatoid arthritis (RA), ulcerative colitis (UC), or uveitis (UV)
  • Resident of the U.S.
  • Undergraduate or graduate student
  • Have completed high school or obtained a General Education Development (GED) certificate.
  • Seeking a professional certification, associate, bachelor's, or graduate degree
  • Fulltime student
  • Planning to enroll at an accredited 24year college, university, or vocational-technical school
